Kamaji held on to his tickets for fourty years – don't know how long mine will last, but we'll see.

Supplies

  • Paper stock
  • printer
  • scissors or cutter
  • ruler
  • cutting mat

I redrew the ticket based on a screengrab. Translations came from this reddit.

It's not 100% acurate, but since everything was hand-drawn, it hardly can be...

The PDF with bleed and cutting marks are attached.

Just print and cut along the marks. Depending on your paper you might want to slightly score the dashed lines to be able to fold it better. Tear off the fifth (blank) ticket at the bottom.

Distress to taste. (again, these are fourty years old)
